WHEN YOU REFER TO T2T CLINIC, PLEASE REMEMBER THE FOLLOWING STEPS:
1. When you do referral please use smartset called .t2t order (Owner Dr. Glatt) which includes the following:
A. specific type of Insulin
B. starting dose for patients
C. hypoglycemic precautions
D. insulin adjustment based on sugar levels
2. Remind your nurse to double book patient because teaching takes an 1hour.
3. Please order supplies prior to visit: (needles, insulin, pens, lancets, strips) because our protocol has not allowed us to order any of these at this moment.
